Propensity score matching (PSM) was used to balance the characteristics of patients who received PORT and those who did not. The final cohort consisted of 81 patients who received PORT and 81 who did not, with balanced characteristics.<br /><br />The analysis showed that pN stage, PORT, serum NSE, and CA125 levels were independent risk factors for survival rates in C-SCLC. Overall, the median disease-free survival (DFS) and overall survival (OS) for the entire cohort were 36.8 and 52.4 months, respectively. Survival analysis showed that patients who received PORT had a more favorable OS both before and after PSM.<br /><br />Further analysis based on the pathological pN stages showed that PORT had no effect on patients with positive lymph nodes. However, it significantly improved DFS and OS in patients with pN1 and pN2 stage C-SCLC.<br /><br />In conclusion, the findings suggest that PORT provides survival benefits for C-SCLC patients with positive lymph nodes after surgery. This study may help guide clinical decision-making regarding PORT in C-SCLC patients.
