EP13.04. Preliminary Results of a Prospective Trial of Hypofractionated Radiotherapy Versus Hyperfractionated Radiotherapy for Limited-stage SCLC - PDF(Slides)

A prospective randomized trial was conducted to compare the effects of hypofractionated radiotherapy and hyperfractionated radiotherapy on limited-stage small cell lung cancer (SCLC) patients. The primary objective was to compare overall survival (OS) between the two treatment groups, while secondary objectives included evaluating progression-free survival (PFS), adverse events, and safety profiles. The study included patients who had histologically or cytologically confirmed limited-stage SCLC and met the enrollment criteria. Patients were randomly assigned to either the hypofractionated or hyperfractionated radiotherapy group. Chemotherapy was administered to both groups, and efficacy was evaluated through CT scans. Acute and advanced toxicities were evaluated using CTCAE5.0. A total of 74 patients were enrolled in the study, with 42 in the hypofractionated group and 32 in the hyperfractionated group. The median follow-up time was approximately 28.6 months and 23.6 months, respectively. The EP chemotherapy regimen was used in the majority of cases in both groups, and prophylactic cranial irradiation (PCI) was used in a high percentage of patients. The results showed a trend towards improved prognosis with hypofractionated radiotherapy compared to hyperfractionated radiotherapy. There were no significant differences in the incidence of grade 3 and above hematological toxicity or radiation pneumonitis between the two groups, but the hypofractionated group had a significantly lower incidence of grade 3 radiation esophagitis. Overall, the study suggests that hypofractionated radiotherapy may be a viable treatment option for limited-stage SCLC patients, warranting further investigation.
