Pdf Summary
This study aimed to determine the immune activation effect of different irradiated sites and its impact on overall survival in patients with extensive-stage small cell lung cancer (ES-SCLC) treated with radioimmunotherapy. The researchers collected data from 45 patients who had received radiation to a primary or metastatic solid tumor. They analyzed changes in blood cell counts and derived blood index ratios before, during, and after radiotherapy.

The results showed that certain blood indexes, such as neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and systemic immune-inflammation index (SII), were correlated with different irradiated organs. They found that the medians of the changes in these blood indexes (delta-NLR, delta-PLR, and delta-SII) were the lowest in the brain irradiated group during immunotherapy.

In terms of overall survival, the researchers found that radiotherapy to the brain had the strongest immune activation effect compared to other organs and was associated with better patient survival during immunotherapy. They also observed that second-line immunotherapy combined with radiotherapy achieved better treatment outcomes.

Univariate analysis was performed to evaluate the associations between various clinical characteristics, inflammatory parameters, and overall survival. However, no statistically significant correlations were found.

In summary, this real-world analysis suggests that radiotherapy to the brain has the greatest immune activation effect and is associated with improved overall survival in patients with ES-SCLC during immunotherapy. Further research is needed to validate these findings and explore the potential benefits of combining radiotherapy with immunotherapy in this patient population.
