EP14.01. Patients Who Undergo Surgery Survive Longer than Those Who Refuse Surgery - PDF(Abstract)

This study aimed to determine whether surgery improves survival outcomes for patients with malignant pleural mesothelioma (MPM) compared to those who refuse surgery. The researchers analyzed data from a prospective database of MPM patients who underwent neoadjuvant chemotherapy (NAC) between 2011 and 2021. Patients who had resectable MPM and had shown effectiveness and tolerability of NAC were considered eligible for curative-intent surgery. Patients who met the eligibility criteria but refused surgery were classified as the refusal of surgery (ROS) group.<br /><br />Of the 337 registered patients, 41 were excluded, leaving 296 eligible for surgery. Among the eligible patients, 272 proceeded with surgery, while 24 refused surgery. The most common surgical procedure was pleurectomy/decortication (75% of cases), followed by extrapleural pneumonectomy (15.8%) and exploratory thoracotomy (9.2%). The median follow-up period was 28.4 months.<br /><br />The results showed that patients who underwent surgery had significantly longer overall survival (median of 40.7 months) compared to those who refused surgery (median of 23.6 months). Similarly, progression-free survival was also significantly longer in the surgery group (median of 20.2 months) compared to the ROS group (median of 12.9 months).<br /><br />This study provides evidence that surgery is associated with improved survival outcomes for MPM patients compared to those who refuse surgery. These findings highlight the importance of considering surgery as a treatment option for eligible patients and the potential benefits it can provide in terms of overall and progression-free survival.
