A study was conducted on patients with malignant pleural mesothelioma (MPM) to compare the outcomes between those who underwent surgery and those who refused surgery. The study included 337 patients who were eligible for surgery after neoadjuvant chemotherapy. The patients who refused surgery were evaluated as the refusal of surgery (ROS) group. The overall survival (OS) and progression-free survival (PFS) were significantly longer in patients who underwent surgery compared to those who refused surgery. The study used the Kaplan-Meier method and Generalized Wilcoxon Test for statistical analysis.<br /><br />The surgical group consisted of 272 patients, with 204 undergoing pleurectomy/decortication (P/D), 43 undergoing extrapleural pneumonectomy (EPP), and 25 undergoing exploratory thoracotomy (ET). The surgery group had a median operation time of 450.5 minutes and a median blood loss of 1380 grams. Grade IIIB complications were observed in 13.2% of P/D cases, 34.9% of EPP cases, and 4.0% of ET cases. The reoperation rate was 11.4%. The median postoperative hospital stay was 23 days. The 30-day mortality rate was 0.4% and the 90-day mortality rate was 2.2%. The pathological IMIG stage distribution in the surgical group was 57 for stage 1A, 73 for stage 1B, 17 for stage 2, 90 for stage 3A, and 35 for stage 3B.<br /><br />The study concluded that patients who undergo surgery for MPM have longer OS and PFS compared to those who refuse surgery. However, it should be noted that the study had limitations such as its retrospective nature and the small sample size in the ROS group. Further research is needed to confirm these findings and determine the optimal treatment approach for MPM patients.
