EP14.01. Risk Factors Influencing Postoperative Pleural Empyema in Patients with Pleural Mesothelioma after Multimodal Therapy - PDF(Abstract)

This study aimed to analyze the preoperative factors that may increase the risk of postoperative empyema in patients with pleural mesothelioma (PM) after multimodal therapy. A retrospective analysis was conducted on PM patients who underwent surgery at the University Hospital of Zurich from 1999 to 2023. Uni- and multivariable logistic regression was used to identify independent predictors of postoperative empyema within 90 days after surgery. <br /><br />Out of the 405 PM patients included in the analysis, 50 developed empyema within 90 days postoperatively (12.3%). Baseline demographics were similar between patients with and without empyema. Of the patients who underwent extrapleural pneumonectomy (EPP), 22.6% developed postoperative pleural empyema within 90 days. Only 5.9% of the remaining patients who underwent other surgical treatments resulted in postoperative empyema. In the multivariable logistic regression analysis, EPP was identified as the sole independent predictor of postoperative empyema.<br /><br />The median overall survival was similar between patients with empyema and those without. The study concluded that patients undergoing EPP had a significantly higher chance of developing pleural empyema within three months after the operation compared to patients undergoing other surgical treatments. No single risk factor was found to influence the occurrence of empyema. These findings support the current standard of care set by the ERS/ESTS guidelines.
