EP14.02. Atypical Response to Metronomic Chemotherapy in a Patient with Advanced Thymoma - PDF(Abstract)

This abstract presents a case report of a patient with advanced thymoma who experienced an atypical response to metronomic chemotherapy. The patient, a 66-year-old with stage IVA Masaoka-Koga thymoma, initially received platinum-based chemotherapy but had fast progression. They were then admitted to the hospital due to worsening symptoms and dyspnea. Metronomic chemotherapy, consisting of daily oral cyclophosphamide and weekly intravenous 5 fluorouracil, vincristine, and liposomal doxorubicin, was initiated during hospitalization. After 4 cycles, the patient experienced grade 3 mucositis and was readmitted to the hospital. Once they recovered, oral cyclophosphamide was withdrawn and the treatment frequency was changed. In April 2022, a major response was achieved, but the patient developed disease progression in March 2023. The patient underwent re-biopsy and NGS analysis, with pending results.<br /><br />The abstract emphasizes the unexpected and long-lasting response to the metronomic chemotherapy regimen. It also highlights the need for further research and collaborative studies to improve treatment options for rare tumors like thymoma. The potential utility of re-biopsy and NGS analysis for unexpected clinical cases is also discussed. The authors suggest that metronomic chemotherapy, particularly without liposomal doxorubicin, could be an alternative for frail patients with thymoma who have failed front-line chemotherapy. Overall, this case report underscores the importance of investigating unconventional treatment approaches for patients with thoracic tumors like thymoma.
