Solitary fibrous tumor of the pleura (SFTP) is a rare tumor found in the pleural cavity. Fibroblast activation protein (FAP) has been identified as a potential target for diagnosing and treating tumors. The usefulness of 18F-labeled FAP inhibitor PET imaging for solid tumors has been reported, including a case of SFTP. This study aims to correlate the imaging results of 18F-labeled FAPI-PET with histological FAP expression in SFTP cases. Thirteen patients who underwent SFTP surgical resection were included in the study. FAP expression in SFTPs was evaluated using immunohistochemical staining. FAP was found to be consistently expressed in soft tissue tumors, including SFTPs. The degree of FAP expression varied among cases. The H-score for FAP in the tumor on 18F-labeled FAPI PET imaging was moderate in all cases, but there were cases with higher or lower H-scores. The study highlights the need for caution when interpreting FAPI-PET imaging results for SFTP diagnosis and differentiation from malignant tumors. Details of patients' information, pathological findings, and FAP expression levels are provided in Table 1. Metastatic lesions or postoperative recurrence were not observed during the follow-up period. FAP expression was observed in all cases, with a median H-score of 160. The study concludes that FAP is expressed in SFTPs and that there is a correlation between histological FAP expression and the image of 18F-labeled FAPI PET in SFTP cases. However, caution is needed when interpreting FAPI-PET imaging results for SFTP diagnosis.
