EP14.03. Lung Epithelioid Hemangioendothelioma with Paraneoplastic Nail Clubbing and Hypertrophic Osteoarthropathy - PDF(Abstract)

This abstract presents a case report of a rare vascular tumor called lung epithelioid hemangioendothelioma. The tumor, which is infrequently found in the lung, is considered a low- to intermediate-grade vascular neoplasm. The authors describe a case of a 35-year-old woman who presented with persistent cough, nail clubbing, severe joint pain, and lower extremity swelling. Imaging tests including bone scintigraphy, CT, and F-18 FDG PET/CT were conducted throughout the course of her disease. The initial bone scan showed features of hypertrophic osteoarthropathy, raising suspicion of lung malignancy. Subsequent CT and PET/CT scans revealed multiple pulmonary nodules with avidity for F-18 FDG, indicating the presence of the tumor. The patient underwent chemotherapy and radiation therapy, however, the disease progressed with widespread metastases involving the pleura, lung, and bones ultimately leading to the patient's demise. <br /><br />This case highlights the clinical manifestations of nail clubbing and hypertrophic osteoarthropathy along with the CT and PET/CT confirmation of lung lesions. It also emphasizes the aggressive nature of lung epithelioid hemangioendothelioma with widespread metastasis. The authors conclude that this case demonstrates the importance of considering epithelioid hemangioendothelioma as a differential diagnosis when encountering patients with nail clubbing and hypertrophic osteoarthropathy. The findings of this case contribute to the understanding of the clinical presentation and behavior of this rare thoracic tumor.
