This case report discusses the successful treatment of a rare type of lung cancer called pulmonary blastoma using immunotherapy and targeted therapy. The patient, a 35-year-old woman, presented with symptoms of cough and sputum and was found to have a tumor in her left lower lung. The tumor was also found to have spread to the lymph nodes and a tumor thrombus was present in the left atrium. The patient was diagnosed with a malignant tumor with a fetal adenocarcinoma component.<br /><br />The patient opted for immunotherapy and targeted therapy instead of conventional chemotherapy and radiotherapy. Treatment with a programmed cell death protein 1 (PD-1) inhibitor and a multi-target tyrosine kinase inhibitor resulted in a partial response. Subsequently, the patient underwent surgery and a lobectomy with lymph node dissection was performed.<br /><br />Pathological examination showed a significant reduction in tumor size and the presence of immune-related pathologic response. The diagnosis of pulmonary blastoma with major pathological response was achieved. The patient is currently receiving postoperative maintenance therapy and there have been no signs of tumor recurrence in subsequent CT scans.<br /><br />Pulmonary blastoma is a rare form of lung cancer characterized by both mesenchymal and epithelial components. There is no standard treatment for pulmonary blastoma, but this case report suggests that a combination of immunotherapy and targeted therapy may be a feasible and effective treatment option. Surgical resection is an important component of therapy for early diagnosed cases.<br /><br />In conclusion, this case report highlights the successful treatment of pulmonary blastoma using immunotherapy and targeted therapy. Further research and clinical data are needed to determine the best treatment strategies for this rare type of lung cancer.
