This document is a case report about a 31-year-old female patient who was diagnosed with dermatofibrosarcoma protuberans (DFSP), a rare cutaneous soft tissue sarcoma of the skin. The patient underwent multiple surgeries to remove the tumor with negative surgical margins and had no evidence of recurrence until November 2022 when she experienced shortness of breath and coughing. A computed tomography (CT) scan and lung biopsy confirmed the presence of lung metastases in December 2022. The patient underwent surgery in February 2022 to remove the lung metastasis, and the final pathological examination revealed a malignant mesenchymal tumor. The surgical margins were negative, and the patient is currently being followed by surgical oncology with plans for possible repeat chemotherapy. The authors suggest that although DFSP has little metastatic potential and usually arises after local recurrences, careful evaluation is needed for early diagnosis and proper management of patients with distant metastasis, especially in cases with hostile histopathologic results.
