Primary pulmonary synovial sarcoma is a rare and aggressive malignant tumor that arises from the mesenchymal tissues of the lung. It accounts for less than 0.5% of all pulmonary malignancies and has a slight male predominance. The diagnosis is usually made through bronchoscopic biopsy. There is no standard recommendation for the optimal management of this condition, but early resection of the tumor followed by chemoradiation is the standard of care. Synovial sarcoma is relatively chemosensitive and responds well to adriamycin alone or in combination with ifosfamide. The 5-year survival rate is approximately 50%, with outcomes depending on tumor grade. Negative prognostic indicators include disease stage, age greater than 20 years, male gender, tumor size larger than 5 centimeters, and high mitotic activity.<br /><br />The case presentation described a 58-year-old non-smoker who presented with a persistent cough, pleuritic pain, and hemoptysis. Imaging showed a large necrotic mass in the left lower lobe of the lung, along with lymphadenopathy. Biopsy confirmed a high-grade synovial sarcoma. The patient was treated with doxorubicin and ifosfamide, underwent surgery and radiation therapy, and had no recurrence of the disease at three months.<br /><br />In conclusion, primary pulmonary synovial sarcoma is a rare and aggressive malignant tumor of the lung. Early recognition and treatment are crucial for optimal patient outcomes. The standard of care involves resection of the tumor followed by chemoradiation. Prognostic factors such as tumor grade, stage, age, gender, and tumor size impact survival rates. Further research is needed to establish standardized guidelines for managing this rare malignancy.
