Three cases of mediastinal teratomas resulting in thoracic perforation were reported. <br /><br />Case 1 involved a 14-year-old male who presented with chest pain after being struck by a soccer ball. A low-density area within the tumor was observed on CT scans, and an emergent surgery using VATS was performed. The pathological findings revealed a mature teratoma, and complete resection was achieved.<br /><br />In Case 2, a 40-year-old male visited the emergency department with chest pain and dyspnea. CT scans showed a giant mediastinal tumor, and the patient developed shock after admission. An emergent operation using VATS was performed, and pus culture indicated MSSA infection. The final diagnosis was a mature teratoma.<br /><br />Case 3 involved a 28-year-old female who complained of upper back pain and cough. CT scans revealed a giant tumor in the right anterior mediastinum. An emergent operation using VATS was performed to alleviate symptoms. The pathology findings confirmed a mature teratoma, and complete resection was performed later.<br /><br />In all cases, the patients experienced different symptoms and underwent emergent operations to alleviate symptoms and control complications. The two-step surgical strategy was employed, with the first operation aimed at alleviating symptoms and making a diagnosis, and the second operation performed for complete resection of any residual tumor.<br /><br />It is important to differentiate between cases with and without thoracic perforation, as the therapeutic strategy may vary. In cases of ruptured tumors, emergent surgeries should be considered for severe or life-threatening symptoms. These patients should be closely followed up to monitor their postoperative courses.
