The study analyzed paired samples of malignant pleural mesothelioma (MPM) patients to investigate the changes in expression of tumor-infiltrating lymphocytes (TILs) and gene expression patterns after treatment with chemotherapy or immunotherapy. The MPM tumor genome is characterized by tumor suppressor loss and low mutational burden. The researchers examined 20 matched tissue samples from 10 patients, taken at diagnosis and at disease progression, including both untreated and treated samples. They quantified the densities of key immune cells using immunohistochemistry and performed gene expression analysis.<br /><br />The results showed that after systemic treatment, there was a decrease in the number of TILs and downregulation of immune-related genes. The analysis of genomic changes revealed 94 differentially expressed genes, with 82 genes being downregulated and 12 genes being upregulated. Some of the highlighted genes included CXCL13, FCRL1, FCMR, FCRL2, BLK, CCR7, FCRL3, Pax5, CD19, CXCL9, IDO2, and HLA-DOB.<br /><br />The study suggests that systemic treatment in MPM patients leads to changes in the tumor microenvironment, including a decrease in TILs and downregulation of immune-related genes. The researchers emphasize the need for larger validation cohorts to determine the most effective treatment schedule considering the dynamic changes in immune profile.<br /><br />In conclusion, this study provides insight into the effects of chemotherapy and immunotherapy on the expression of TILs and gene expression patterns in MPM patients. The findings highlight the potential importance of the immune microenvironment in MPM and support further research to optimize treatment strategies based on immune profile dynamics.
