This study focuses on the cardiovascular implications of a lung cancer screening program called the Smokers health Multiple Actions (SMAC) study. The aim of the study was to assess the cardiovascular risk profile of subjects enrolled in a lung cancer screening using low-dose computed tomography (LDCT). <br /><br />The study enrolled 1,112 subjects aged 55 years or older who were current smokers or had a smoking history of at least 30 pack-years and had quit within the past 15 years. All subjects underwent non-contrast LDCT chest scanning for lung cancer screening between 2018 and 2019 at a single tertiary academic hospital. <br /><br />The study found that approximately one-third of the subjects, who had no history of cardiovascular disease, were at high or very high cardiovascular risk. Only a minority of subjects were at low risk and one-third had zero coronary artery calcium (CAC) score. <br /><br />The estimated cardiovascular risk was low in 1.7%, intermediate in 64.4%, high in 23.4%, and very high in 10.5% of the subjects. Furthermore, almost half of the subjects in the entire cohort, and more than half in the intermediate and higher cardiovascular risk categories, had a recommendation for statin initiation. <br /><br />These findings suggest that implementing cardiovascular primary prevention programs in patients undergoing lung cancer screening could help reduce the burden of cardiovascular disease.
