This presentation discusses the results of the baseline round of the Smokers health Multiple Actions (SMAC) trial, which aimed to demonstrate the feasibility and effectiveness of an integrated program for early detection of smoking-related thoraco-cardio-vascular diseases in high-risk subjects, combined with primary prevention. The study focused on the oncological findings of the screening program. <br /><br />The study included high-risk subjects who were defined as being 55 years of age, active smokers or those who had quit within the past 15 years, and had a smoking history of at least 30 pack-years. The subjects underwent a series of tests and evaluations, including a psychology consultation, blood sample collection, self-evaluation questionnaire, spirometry, and low-dose computed tomography (LDCT) scan. Blood samples were analyzed for various markers, and cardiovascular risk scoring was performed. <br /><br />Out of 1654 eligible subjects, 1112 participated in the study. A total of 344 subjects had at least one lung nodule, and 31 cancers were diagnosed, with 29 being lung cancers. The LDCT scans had a very low rate of false positives and allowed for the safe detection of early-stage lung cancer in high-risk individuals. The study also found that by re-calculating the lung cancer risk using the Maisonneuve model, the screening time-interval could be optimized. <br /><br />The findings of this study support the efficacy of LDCT screening in detecting early-stage lung cancer with minimal false positives. The results also provide valuable information for policymakers in terms of optimizing screening programs and improving outcomes for lung cancer patients.
