P1.18. Systematic Enrolment Ensures High Participation Rate in Lung Cancer Screening - Results of Regional Pilot Study in Estonia - PDF(Abstract)

The aim of the study was to evaluate the results of a systematic patient enrollment method for lung cancer screening conducted in Estonia. The study was a regional pilot project that enrolled participants through family physicians or nurses who assessed their patients to determine potential target group patients. The inclusion criteria for the screening included smoking status and lung cancer risk score. Participants with elevated risk were given low-dose computed tomography (LDCT) scans. The scans were evaluated independently by two radiologists using the LungRADS version 1.1 system.<br /><br />In total, there were 137,181 patients in 74 participating family physician practices, of whom 26,759 belonged to the target age group. The enrollment criteria of 24,415 patients were evaluated, and out of these, 17,217 patients had exclusion criteria. The remaining 7,198 patients met the inclusion criteria and were referred for LDCT scans. A total of 3,444 patients (92.9%) underwent LDCT. The overall participation rate in the screening was 79.3% for the target group.<br /><br />There were 28 confirmed cases of lung cancer, with 11 cases in stage I and 18 cases treated surgically. Incidental findings were detected in 96.9% of patients who underwent LDCT, with the most common findings being coronary atherosclerosis and emphysema. Additionally, 12 other malignant tumors were diagnosed, with kidney cancer being the most common.<br /><br />The study concludes that systematic enrollment for lung cancer screening by family physicians or nurses is feasible and ensures a high uptake in the target group. The screening results and incidental findings were comparable to studies conducted in other countries. The data from the pilot study provide valuable input for planning a national lung cancer screening program in Estonia and other countries with similar healthcare systems.
