A study conducted by Young Kwang Chae, MD, MPH, MBA, examined the potential of immune cells present in the tertiary lymphoid structures (TLS) surrounding tumor sites as predictors for immunotherapy. The study utilized AI technology to assess TLS in H&E whole-slide images and determine its correlation with immune phenotype and immunologic signatures.<br /><br />The study included 85 patients with advanced-stage non-small cell lung cancer (NSCLC) who were treated with either immune checkpoint inhibitor (ICI) monotherapy or a combination of ICI and chemotherapy. The presence of TLS was analyzed using the Lunit SCOPE IO, an AI-powered H&E WSI analyzer.<br /><br />Results showed that 29.4% of the samples contained TLS. The presence of TLS was associated with favorable survival outcomes in terms of overall survival (OS) and progression-free survival (PFS). However, higher TLSP (TLS proportion within the tumor microenvironment) did not correlate with improved OS or PFS.<br /><br />The study also found that the presence of TLS was significantly decreased in patients with KRAS mutations. The PD-L1 tumor proportion score did not seem to affect the predictive role of TLS on survival outcomes.<br /><br />No significant difference was observed in the pre-treatment neutrophil-lymphocyte ratio or tumor mutational burden between samples with TLS present or absent.<br /><br />In conclusion, the study suggests that the presence of TLS in the tumor site may serve as a favorable predictor for immunotherapy in advanced-stage NSCLC patients. AI technology can be a valuable tool in assessing TLS in H&E whole-slide images and predicting immune phenotypes and signatures. Further research is warranted to explore the potential of TLS as a biomarker for immunotherapy response.
