This study examines the role of tertiary lymphoid structures (TLS) in predicting disease-free survival (DFS) in patients with resected lung adenocarcinoma harboring sensitizing EGFR mutations. It is known that EGFR mutant lung cancer is the most common subtype among all lung adenocarcinoma cases in Asia, and patients with EGFR mutations have a higher rate of recurrence after surgical resection compared to those with EGFR wild type. Therefore, identifying predictors of disease recurrence is crucial in order to stratify high-risk patients for more intensive adjuvant treatments and follow-ups. <br /><br />TLS has been associated with survival and treatment responses in various cancer types, and higher B cell density in the tumor microenvironment has been linked to better treatment response to TKI in advanced status. The study aims to evaluate the role of TLS in resected EGFR mutant lung cancer. Tumor microenvironment was analyzed using multiplex immunohistochemical staining and specific phenotypes of tumor infiltrating immune cells were identified. TLS was defined as CD20 B cells aggregation and follicular dendritic cells surrounded by T cells. The number of TLS was counted and correlated with clinical outcomes. <br /><br />The results showed that patients with a high number of TLS had longer DFS compared to those with a low number of TLS, regardless of EGFR mutation types, stages, gender, and PD-L1 expression. This suggests that TLS can be used as a biomarker to guide clinicians in tailoring adjuvant treatment after tumor resection. However, further investigation is needed to understand the function of TLS in early EGFR mutant lung cancer.
