A study presented at the WCLC 2023 conference discussed the correlation between levels of phosphorylated PD-L1 in extracellular small vesicles (EV pPD-L1) and Tumour Proportion Score (TPS) in non-small cell lung cancer (NSCLC) patients. The authors aimed to determine whether EV pPD-L1 levels could be used as a liquid biopsy biomarker to guide the use of PD-1 and PD-L1 immune checkpoint inhibitors (ICI) in NSCLC.<br /><br />The study analyzed plasma samples from NSCLC patients prior to therapy and measured EV pPD-L1 and PD-L1 levels. EV pPD-L1 levels were assessed using a nanotechnology chip-based assay that detected changes in current between EV pPD-L1 and non-pPD-L1. Plasma PD-L1 levels were measured using an ELISA.<br /><br />The results showed a linear correlation between EV pPD-L1 levels and TPS, indicating that EV pPD-L1 levels could be used to select patients for PD-1 and PD-L1 ICI therapy. In contrast, plasma PD-L1 levels did not correlate with TPS.<br /><br />The authors suggest that EV pPD-L1 levels could be used to guide the use of PD-1 and PD-L1 ICI therapy in early, locally advanced, and advanced NSCLC. They also highlight the reproducibility and relatively short turnaround time of the nanotechnology chip-based assay, which could facilitate its adoption in routine diagnostic laboratories.<br /><br />This study contributes to the ongoing research on liquid-based biomarkers in NSCLC and provides evidence for the potential use of EV pPD-L1 levels as a predictive biomarker for PD-1 and PD-L1 ICI therapy. Further randomized controlled trials are needed to validate these findings.
