The researchers aimed to determine whether adjuvant concurrent chemoradiation therapy (CCRT) improved survival outcomes compared to adjuvant chemotherapy (CTx).<br /><br />The study analyzed 210 patients with ECE-positive NSCLC who underwent upfront surgery between 2008 and 2016. The patients were divided into two groups based on the type of adjuvant therapy they received: CCRT or CTx. The researchers compared recurrence-free survival (RFS) and overall survival (OS) between the two groups.<br /><br />Out of the 210 patients, 130 (62%) received adjuvant CCRT and 80 (38%) received adjuvant CTx. During a median follow-up of 50.5 months, 115 patients experienced recurrences and 90 patients died. The 5-year RFS rate was 61.4% in the CCRT group and 52.5% in the CTx group, while the 5-year OS rate was 55.1% in the CCRT group and 63.0% in the CTx group. However, there was no statistically significant difference in survival outcomes between the two groups.<br /><br />Further analysis showed that the CCRT group had a lower locoregional recurrence rate (7.7%) compared to the CTx group (17.5%), although there was no significant difference in recurrence patterns. Overall, adjuvant CCRT did not provide a significant improvement in survival outcomes compared to chemotherapy in patients with ECE-positive stage III-N2 NSCLC.<br /><br />The findings of this study suggest that adjuvant CCRT may not be necessary in these patients, and adjuvant chemotherapy alone may be sufficient. This information can help guide treatment decisions for ECE-positive NSCLC patients who have undergone upfront surgery.
