A pan-European expert panel conducted a clinical case review to define resectable stage III non-small cell lung cancer (NSCLC). The goal was to reach a uniform definition of resectability for this heterogenous disease. The review included a consecutive series of patients with stage III NSCLC diagnosed between 2016 and 2021. An imaging platform was used to upload anonymized chest-CT/PET-CT images and provide information from various diagnostic procedures. There were three rounds of review, during which participants filled in a questionnaire to determine if a tumor was considered resectable upfront and, if yes, the type of surgery. Consensus was reached for most T-N categories, except for T4-invading/N0 and some cases of N2 multi-station involvement. T3-T4 due to size/satellite nodules and N1-N2 single-station cases were considered resectable, while T4-invading tumors were mostly deemed unresectable, especially if combined with N2-N3 disease. Tumors invading the esophagus or spinal cord were unanimously defined as unresectable. Some cases of involvement of the heart and vertebra were considered resectable conditional on evaluation in specialized centers. N2-multi-station/bulky and N3 involvement were generally considered unresectable. Further discussion is warranted for subgroups with limited case evaluations. The study included 105 consecutive stage III NSCLC cases, with the final analysis including 104 cases after three rounds of review. The results provide a clearer definition of resectable stage III NSCLC, which is important for treatment decision-making and patient outcomes.
