The presentation aimed to explore how patient characteristics, tumor characteristics, and treatment history impact survival outcomes in the context of evolving treatment paradigms for mesothelioma. <br /><br />The study conducted a retrospective analysis on all patients referred to BC Cancer between January 2010 and December 2022 who were diagnosed with pleural mesothelioma or received dual anti-PD-1 plus anti-CTLA-4 immunotherapy. The baseline characteristics of the cohort were summarized, and overall survival (OS) was calculated and compared using the Kaplan-Meier method and log-rank test. Multivariate analysis was also conducted using Cox-regression analysis.<br /><br />The results showed that systemic therapy had benefits for patients with unresectable pleural mesothelioma regardless of performance status (PS) and histology. The median OS was 7.2 months with best supportive care only, 15.0 months with first-line platinum doublet chemotherapy, 22.3 months with immunotherapy, and 8.0 months with single-agent chemotherapy. Univariate and multivariate analyses for OS indicated that ECOG 0-1, epithelioid histology, and first-line platinum doublet or immunotherapy were associated with better outcomes.<br /><br />In conclusion, this real-world study demonstrated the benefits of systemic therapy for patients with unresectable pleural mesothelioma. The early results of immunotherapy implementation in the first-line setting were promising and suggest hopeful outcomes for these patients.
