Pdf Summary
This study examined the clinical outcomes of patients with thymic carcinoma who underwent preoperative induction therapy followed by surgical resection. The researchers reviewed the cases of 16 patients who underwent surgical resection at Nagoya University Hospital between 2008 and 2022. The median age of the patients was 56 years, with 9 males. The histologic type of the carcinoma was squamous cell carcinoma or LCNEC. The majority of patients (13 out of 16) had stage IIIa or higher disease. Preoperative treatment included chemoradiotherapy, radiotherapy alone, or chemotherapy alone, with 10 patients showing stable disease or partial response. All patients had a thymectomy and concomitant resection of adjacent organs. The majority of patients (13 out of 16) achieved R0 resection, with 3 patients having a major pathological response. Postoperative complications were relatively low, with only 3 patients experiencing grade III or IV complications. The median overall survival was 61 months, with a 5-year survival rate of 84%. The median recurrence-free survival was 31 months, with a 5-year recurrence-free survival rate of 39%. Recurrence occurred in 9 patients, with 3 cases of local recurrence and 6 cases of distant metastatic recurrence. The researchers concluded that surgery after preoperative induction therapy for thymic carcinoma with adjacent organ invasion can be performed safely and may result in long-term recurrence-free survival.
