Pdf Summary
This document discusses the implementation of a novel chart rounds application for peer review of lung radiotherapy plans in a virtual environment. Peer review is an important aspect of quality assurance and safety in radiotherapy treatments for lung cancers. The new application, called novel chart rounds (NCR), was developed by a working group at Emory University and implemented at a multi-site academic center in the United States.

The NCR format includes several critical components. Firstly, it divides the review sessions by disease site, with a specific focus on stereotactic body radiotherapy (SBRT) and lung plans. This allows for more time to be spent on each patient and facilitates more robust discussions. Secondly, the application includes a standardized plan scoring system to record feedback. Thirdly, it allows for the recording of review and feedback within the application itself. Lastly, there is a mechanism for timely and reliable relay of critical feedback to the treating providers.

The initial implementation of the NCR application has shown promising results. Compared to the previous standard chart rounds (SCR) format, the NCR format allows for more plans to be reviewed per hour on average. Additionally, the NCR application automates the selection of plans requiring peer review, reducing the chances of plans being missed or excluded. In the first 10 months of use, 327 plans have been reviewed, with some requiring minor changes and others requiring major changes.

The NCR format and application address the challenges of performing peer review in large institutional, multi-site, and virtual settings. It enables efficient and accurate review of radiation plans in the virtual setting, utilizing features such as visualization of 4D scans. The feedback from the scoring system is incorporated into the physician workflow to ensure that necessary changes are addressed.

Overall, the NCR format and application offer a solution for effective peer review of lung radiotherapy plans in various complex planning scenarios. It has the potential to be implemented in other institutions worldwide.
