The previous standard chart rounds (SCR) format had several challenges, including manual case input resulting in duplicate and missed cases, difficulty in reviewing all new cases within the given time frame, glitches in the application, lack of standardized review, and lack of productive educational discussions.<br /><br />To address these challenges, a working group was created to develop the NCR format and application, which was implemented in May 2022. The application was further customized based on real-world use. The results of the implementation showed that the NCR format and application enabled efficient and reliable peer review of radiotherapy plans across a variety of complex planning scenarios.<br /><br />The NCR format consisted of six disease site-specific weekly sessions, with each session lasting for 60 minutes. On average, 11 plans per hour were reviewed using the NCR format, compared to 49 plans per hour with the previous SCR format. The NCR application reduced preparatory time through automation and developed a searchable database of cases. It also included unique features such as incorporating 4D scans for motion visualization, MRI registrations, and the ability to view composite plans. The application flagged unmet clinical goals in red and automatically marked abnormally shaped structures.<br /><br />Scoring and feedback were standardized within the application, with plans being scored as requiring no changes, minor changes, or major changes. Major changes triggered a replan, and feedback from the scoring was incorporated into the physician workflow.<br /><br />Overall, the implementation of the NCR format and application improved the efficiency and reliability of the peer review process for lung radiotherapy plans. It addressed the challenges faced with the previous SCR format and provided unique features to enhance the review process.
