Pdf Summary
This study explores the incidence and survival outcomes of post-double lung transplant lung cancer (PTLC) and its association with graft rejection. The research used data from the United Network for Organ Sharing/Organ Procurement and Transplant Network (UNOS/OPTN) database from 1987 to 2022. The study found that double lung transplant recipients have a higher risk of developing lung cancer compared to the general population.

Some key findings include:
- Among 21,106 double lung transplant recipients, 0.8% developed PTLC, and 0.02% had both PTLC and rejection.
- Factors associated with higher odds of PTLC include age over 56, female sex, smoking, and blood group AB.
- PTLC is a risk factor for graft failure, with a higher risk observed in the first 1-3 years post-transplant.
- Median overall survival of PTLC was 52 months, and the 5-year overall survival rate was 35.6%.
- The study found no statistically significant difference in graft failure-free survival between recipients with PTLC and those without PTLC.

The study highlights the significance of malignancy and graft rejection as complications following double lung transplantation. It also emphasizes the potential increased risk of lung cancer in recipients with native lungs and the impact of immunosuppressive therapies on cancer risk. The research provides a comprehensive overview of PTLC and its relationship with graft failure using a large multicenter database.

Overall, this study contributes to our understanding of the incidence, outcomes, and risk factors associated with post-double lung transplant lung cancer. The findings can help inform clinical management and surveillance strategies for double lung transplant recipients to improve long-term outcomes.
