The Korean National Lung Cancer Screening Program (KNLCS), started in 2019, utilizes low-dose computed tomography (LDCT) to reduce lung cancer mortality among high-risk individuals. This study investigates the distribution of Lung Imaging Reporting and Data System (Lung-RADS) categories over three screening rounds to assess the program's efficiency and potential areas for improvement. <br /><br />Data from 9,631 participants, who underwent at least three rounds of lung cancer screening through the National Cancer Screening Information System (NCSIS), were analyzed. This system, managed by the National Cancer Center Korea, aids with medical expenses and post-screening management. The focus of this analysis was on positive rates (i.e., Lung-RADS categories 3 or 4) and how previous screening results impacted future outcomes.<br /><br />Results indicated a notable decline in the proportion of patients categorized as Lung-RADS 3, decreasing from 4.5% in the first round to 2.9% in the second, and further to 2.5% in the third. However, the proportion of patients placed in category 4 remained relatively constant at approximately 3.6-3.8% across all three rounds.<br /><br />Conclusively, the steady decrease in positive rates (Lung-RADS 3) over time suggests increased screening efficiency in the program as participants continued in subsequent rounds. However, the consistency in category 4 suggests the need for ongoing assessment of program strategies to manage stable high-risk individuals effectively.<br /><br />The eligibility criteria for lung cancer screening include individuals aged 54 to 74 with a history of at least 30 pack-years of smoking, and who are current smokers, with LDCT screenings recommended every two years. The findings from this study will be presented at the 2024 Asia Conference on Lung Cancer in Hong Kong, China Mainland.
