This study aimed to evaluate the feasibility and safety of using the Da Vinci Xi robotic system for single-port surgery in treating early-stage lung cancer. Researchers conducted a retrospective analysis of cases from November 29, 2023, to June 18, 2024, involving 13 single-port lung lobectomies and 27 single-port lung segmentectomies at their hospital. The surgical procedures, techniques, and outcomes were examined.<br /><br />Key findings include an average robotic arm docking time of approximately 12.8 minutes. The surgeries lasted an average of 240.7 minutes for lobectomies and 135.8 minutes for segmentectomies. Intraoperative blood loss averaged 107.5 ml, with chest tubes being retained for about 3.7 days and postoperative hospital stays averaging 8.1 days. The average costs were 49,299 RMB for lobectomies and 42,194 RMB for segmentectomies.<br /><br />On average, 7 lymph node stations were dissected during lobectomies, with about 17.8 lymph nodes removed. Pathological results post-surgery showed 32 cases of lung adenocarcinoma and 8 of lung squamous cell carcinoma, with pathologic staging aligning with clinical expectations.<br /><br />The study concluded that the Da Vinci Xi system offers flexibility for early-stage lung cancer surgeries but acknowledges that the limited space in single-port operations restricts robotic arm movements. While the system aids in precise small-scale operations such as segmental surgeries, more clinical experience is essential to optimize positioning and movement of the robotic arms. The study also highlights that the use of multifunctional Maryland forceps can reduce the number of robotic arms needed, minimizing medical burdens on patients.
