This study, conducted by a team from the Seoul National University, evaluates the feasibility of wedge resection in patients with peripheral clinical T1bN0M0 non-small cell lung cancer (NSCLC) over a decade (2011-2022). A total of 423 patients underwent wedge resection, a form of sublobar resection. This approach was inspired by the CALGB 140503 study which previously found sublobar resection to be noninferior to lobectomy for peripheral clinical T1b NSCLC, and noted that a substantial percentage of sublobar surgeries were wedge resections.<br /><br />The patient demographics indicate an average age of 62 years, with a nearly even male representation and around 42% having a history of smoking. The majority of tumors were adenocarcinomas, with other histologies including squamous cell carcinoma. Tumor characterization showed a mixture of solid nodules, part-solid nodules, and pure ground-glass nodules.<br /><br />Results showed outstanding overall survival rates for all sub-stages (T1mi, T1a, and T1b), with some recurrence noticed particularly in non-T1mi stages. The study identified no significant difference in the overall survival rates among the subtypes. However, both cT1a and cT1b demonstrated higher local recurrence (LR) rates compared to cT1mi. Regarding post-resection lung function, a small but statistically significant decline was observed in forced vital capacity (FVC) and forced expiratory volume (FEV1) after surgery.<br /><br />In conclusion, this research supports wedge resection as a feasible surgical option for treating peripheral cT1b NSCLC, providing excellent overall survival rates despite a higher recurrence rate in certain sub-stages. The findings underscore the potential of wedge resection as an alternative surgical strategy, potentially broadening treatment options for lung cancer patients.
