The study analyzed the outcomes of advanced non-small cell lung cancer (NSCLC) patients with MET exon 14 (MET ex14) skipping mutations treated at a hospital from June 2020 to April 2024. It focused on comparing the effectiveness of MET tyrosine kinase inhibitors (TKIs) versus immune checkpoint inhibitors (ICIs), including some cases combined with chemotherapy.<br /><br />Patient data included demographics, disease stage, performance status, and the treatments received. The study aimed to determine overall survival (OS) and objective response rate (ORR) among different treatment groups and assess subsequent treatments.<br /><br />53 patients with MET ex14 mutations were reviewed, out of which 42 received MET-TKI (primarily Tepotinib, with two on Capmatinib), and 24 received ICIs. The median age for TKI and ICI groups was 72.5 and 68.5 years, respectively. Many patients had adenocarcinoma histology and were stages 3 or recurrent.<br /><br />PD-L1 expression was notably present in about 50% of tumors, consistent with existing literature. The study found that the ORR for the TKI group was 38.1%, while the ICI group had an ORR of 37.5%. Disease control rates were similar between the groups, with 69% for TKI and 66.7% for ICI.<br /><br />Kaplan–Meier analysis suggested improved OS with ICI therapy compared to TKI therapy. The median OS for the first-line TKI group was 21.3 months, while data for ICI could not be established due to a wide confidence interval but suggested a trend towards longer survival. This implies that initial treatment with ICIs might be a reasonable alternative to TKIs for patients with MET 14-skipping NSCLC based on observed outcomes.
