This study investigates the Neutrophil to Lymphocyte Ratio (NLR) and Platelet to Lymphocyte Ratio (PLR) as prognostic markers for advanced non-small cell lung cancer (NSCLC) with a focus on patients tested for Epidermal Growth Factor Receptor (EGFR) mutations. Conducted retrospectively at a single institution in Thailand, the study evaluates how elevated NLR and PLR levels correlate with patient outcomes.<br /><br />Between January 2018 and December 2022, 80 patients with advanced lung adenocarcinoma were analyzed, with 28 (35%) exhibiting EGFR mutations, predominantly sensitizing mutations (exon 19 deletion and exon 21 L858R). These patients primarily received treatment with first-generation EGFR Tyrosine Kinase Inhibitors.<br /><br />Overall survival (OS) was measured, with different outcomes observed based on NLR and PLR values. The median OS was 19.5 months for patients with EGFR mutations, compared to 8.4 months for those with wild-type EGFR. Stratification by NLR showed that patients with lower NLR (NLR < 3) had better outcomes, with a median OS of 31.0 months, compared to 11.7 months for those with higher NLR (NLR ≥ 3). Similarly, patients with a lower PLR (PLR < 260) had a median OS of 16.5 months, significantly longer than 7.4 months for PLR ≥ 260.<br /><br />Multivariate Cox regression analysis indicated that high NLR (≥ 3) and PLR (≥ 260) are associated with worse overall survival, with hazard ratios (HR) of 2.46 and 4.26, respectively.<br /><br />The study concludes that elevated NLR and PLR are linked to poorer prognoses in advanced lung adenocarcinoma, independent of EGFR mutation status, highlighting their potential role as clinical prognostic markers.
