Pdf Summary
The study focuses on the transformation of non-small cell lung cancers (NSCLC) with EGFR mutations into small cell lung cancer (SCLC), a condition known for its aggressiveness and poor prognosis. This transformation exhibits heterogeneity in response to EGFR-TKIs, time to transformation, survival, and genetic profiles among patients. While it is known that concurrent mutations in EGFR, RB1, and TP53 genes are associated with a higher risk of SCLC transformation, about 80% of patients with these triple mutations do not undergo transformation.

Through a retrospective analysis of EGFR-mutant NSCLC patients treated at the Guangdong Lung Cancer Institute from January 2017 to December 2021, researchers aimed to identify distinct subtypes of SCLC resulting from EGFR-mutant NSCLC transformation, based on the RB1 status. This new classification could point to varying therapeutic opportunities for these subtypes.

The study developed a nomogram model that enhances the prediction accuracy of SCLC transformation risk in patients with mutations in EGFR, RB1, and TP53. The model's efficacy was tested through training and external validation cohorts, demonstrating its potential utility in clinical settings to better assess transformation risk. This model awaits further validation but represents a promising tool for refining treatment strategies for affected patients.

Funding for the research was provided by several health and research institutions, and the authors acknowledge contributions from various professionals and the patients who participated in the study. Such collaborations underscore the importance of pooling knowledge and resources in advancing cancer research and improving patient outcomes.
