This study explores the relationship between gut microbiota, metabolites, and malignant pleural effusion (MPE) in patients with lung adenocarcinoma (LUAD). The research highlights that patients with LUAD and MPE have distinct profiles of intestinal flora and metabolic pathways, which could serve as potential biomarkers for diagnosing and prognosing MPE.<br /><br />The study examined 122 newly diagnosed LUAD patients, dividing them into two groups based on the presence (53 patients) or absence (69 patients) of MPE. Metagenomic and metabolomic analyses were conducted on baseline stool samples collected before treatment to identify differences in gut microbiota and metabolites between the MPE and NPE (non-pleural effusion) groups.<br /><br />Significant differences in gut microbiota composition were found between the groups. The MPE group exhibited a reduced relative abundance of Methanobrevibacter, Flavobacteriales, Flavobacteriia, and Holdemanella_biformis. Conversely, there was an increased relative abundance of Anaeroglobus, Alistipes_ihumii, and Dialister_invisus in the MPE group. Additionally, metabolites such as DL-o-Tyrosine and Brevifolincarboxylic acid were up-regulated in the MPE group.<br /><br />Using the Kyoto Encyclopedia of Genes and Genomes (KEGG) database, the study enriched differential metabolites and discovered significant variations in lipid metabolism-related pathways, including steroid hormone biosynthesis, arachidonic acid metabolism, and fatty acid biosynthesis.<br /><br />These findings suggest a critical link between gut microbiota and metabolites with the development and presence of MPE in LUAD patients. Consequently, the unique intestinal flora and metabolites could serve as novel biomarkers, assisting in the early diagnosis and prognosis of MPE in LUAD, ultimately potentially improving patient outcomes and quality of life.
