This case series explores double lung transplantation (DLT) as a potential treatment for patients with stage IVA non-small cell lung carcinoma (NSCLC) confined to the lungs, where traditional surgical removal might not be viable. The study, conducted at Northwestern University's Canning Thoracic Institute, examined seven patients undergoing DLT from September 2021 to August 2024 as part of the DREAM study, focusing on lung-limited malignancies.<br /><br />The participants, aged 56 to 71, mostly comprised White males, with a significant portion having a history of smoking. These patients had previously undergone a median of three lines of treatment without success, leading to the decision for transplantation due to respiratory failure or refractory cancer.<br /><br />The study found that DLT was performed successfully on all patients without transplant rejection. Post-transplant, no patients required supplemental oxygen, and the majority remained disease-free for the duration of the follow-up period. Only one patient experienced a disease recurrence 18 months post-transplant, which was treated with stereotactic body radiotherapy (SBRT).<br /><br />Histological analysis showed that most patients had invasive mucinous adenocarcinoma, with others having variants such as acinar or mixed histology. Importantly, pre-transplant assessments showed no lymph node involvement, and one patient with a known mutated lung cancer did not respond to targeted therapy before undergoing transplant.<br /><br />The conclusions highlight the potential of DLT as a viable treatment option for stage IVA NSCLC, showing promising survival rates and health outcomes. The DREAM study will continue to assess the long-term efficacy and safety of DLT for this patient group, especially those without significant lymph node involvement (pN0).
