The study investigates the impact of primary tumor resection (PTR) on the survival of patients with metastatic non-small cell lung cancer (NSCLC) with synchronous lung or liver metastases. Utilizing data from 7,660 NSCLC patients retrieved from the SEER database, including both a PTR group (394 patients) and a no-PTR group (7,266 patients), a 1:1 propensity score-matched analysis was conducted to minimize baseline differences. The findings indicate that patients who underwent PTR exhibited significantly improved overall survival (OS) and cancer-specific survival (CSS), with median survival times of 18.0 months and 13.0 months, respectively, for the PTR group, compared to 9.0 months and 7.0 months for non-PTR patients.<br /><br />Despite the improvements observed with PTR, the study notes that lung and liver metastasectomy did not impact survival outcomes. Further multivariable Cox analysis reinforced PTR as a protective factor against mortality with a hazard ratio (HR) of 0.52. Additionally, PTR combined with chemotherapy showed the greatest treatment effect. The pivotal independent risk factors for patients without PTR included being older than 65, male, having a higher N stage (N1-N3), larger tumor size (5 cm), worse tumor differentiation, and the absence of systemic therapy.<br /><br />The research underscores the potential survival benefits of PTR in metastatic NSCLC patients and suggests further exploration through a well-designed prospective randomized controlled trial to confirm these findings.
