P2.04A.06 A Multilevel Approach to Address Disparities in Lung Cancer Screening: The PREP Trial
Back to course
Pdf Summary
The PREP Trial is addressing disparities in lung cancer screening, a crucial issue given that lung cancer is the leading cause of cancer-related mortality in the United States with over 120,000 deaths expected in 2024. Current screening uptake is low, especially among Black/African Americans, who are often diagnosed with advanced disease and have lower survival rates compared to White individuals. The trial aims to improve screening rates through a multilevel approach targeting both patient and provider behaviors.

The study utilizes a quasi-experimental design involving four primary care clinics within the MedStar Health system in the Mid-Atlantic region of the US, with a focus on clinics with significant Black/African American representation. Key eligibility criteria for patient participants include being aged 50-80, with a history of smoking, and non-adherence to lung screening, among others.

The intervention includes provider-level strategies such as sending electronic health record (EHR) pre-visit planning messages to providers, prompting discussions about lung cancer screening with eligible patients. At the patient level, an in-reach specialist educates patients about screening before their clinic visits to improve knowledge and facilitate screening discussions.

The trial is assessing various outcomes, such as the frequency of provider-patient screening discussions, patient screening intentions and knowledge, and actual screening referrals and completion rates tracked via the EHR over six months.

Ultimately, the results from the PREP Trial, funded by the National Cancer Institute and involving contributions from several institutions, will help shape future strategies aimed at increasing lung cancer screening and promoting health equity among minority populations.
