Pdf Summary
This study explores a seven-year lung cancer screening program at a sub-urban community hospital, focusing on low-cost, low-dose CT (LDCT) scans. Lung cancer is a leading cause of cancer-related death in the U.S. To aid early detection, the U.S. Preventive Services Task Force (USPSTF) recommends annual LDCT scans for high-risk individuals, including those aged 50-80 with a significant smoking history. However, cost barriers limit access to these screenings.

The hospital implemented a program to offer affordable LDCT scans, significantly reducing costs, even providing free screenings to some during Lung Cancer Awareness Month. A multi-disciplinary Lung Nodule Review Board (LNRB) consisting of various specialists was established to review findings and communicate recommendations, aiming to improve follow-up care and adherence to guidelines.

Out of 3,095 patients screened over seven years, 6,646 LDCT scans were performed. Most patients (84%) had non-concerning findings and were advised to continue annual screenings, while 16% required further testing. The program detected 162 new cancers, including 135 primary lung cancers, with 64% identified at early stages (Stage 1 or 2). The incidence of lung cancer among screened individuals was 4.36%, reduced to 2.03% when adjusted for patient years.

The program demonstrated a higher detection rate and earlier diagnosis compared to national trials, attributed to lower screening costs, comprehensive patient navigation, and multidisciplinary support. Broad community education and strong primary care involvement were critical to the program's success. As more patients become eligible for screening, the program anticipates growth, emphasizing the importance of cost-effective, comprehensive strategies for lung cancer control, improving both mortality and morbidity outcomes.
