Pdf Summary
This study, conducted at the Shanghai Pulmonary Hospital, investigates clinical presentations, outcomes, and anticoagulant strategies in patients with lung cancer-associated isolated distal deep vein thrombosis (LC-iDDVT). Isolated distal deep vein thrombosis refers to a thrombus in veins below the popliteal vein without coexisting pulmonary embolism (PE). Data were collected retrospectively on 853 patients diagnosed with active primary lung cancer and acute lower extremity DVT from January 2013 to June 2023. Those with concomitant symptomatic PE, upper extremity DVT, or a history of venous thromboembolism (VTE) were excluded.

The study divided iDDVT patients into axial venous thrombosis (AVT) and muscular venous thrombosis (MVT) groups. The aim was to compare clinical presentations and prognosis with those having proximal DVT (LC-proximal DVT). Outcomes, including PE, VTE recurrence, DVT deterioration, major bleeding, and all-cause death, were assessed using logistic regression and survival analysis, covering short-term (90-day) and long-term (1-year) outcomes.

Findings suggest that in lung cancer settings, AVT patients have comparable prognoses to those with LC-proximal DVT but are more prone to recurrent thrombosis, especially if anticoagulation is inadequate. MVT patients showed a lower risk of PE and VTE recurrence at both 90-day and 1-year follow-ups compared to LC-proximal DVT patients. Furthermore, those with AVT had reduced VTE recurrence risk when on anticoagulation therapy for three months. Overall, patients with LC-iDDVT had lower risks of PE and VTE recurrence than those with LC-proximal DVT, highlighting the importance of effective LC-iDDVT management.

While the study underscores AVT's severity in lung cancer patients, the authors note the limitations inherent in retrospective studies and recommend further validation through prospective research.
