P2.05B.01 How Reliable Is PET/CT in T1A and T1B Non-Small Cell Lung Cancer for Mediastinal Staging?

The study examines the reliability of PET/CT for mediastinal staging in patients with T1a and T1b non-small cell lung cancer (NSCLC). Conducted retrospectively at Istanbul University-Cerrahpaşa, the research analyzes data from 224 patients between January 2002 and January 2022. The aim was to assess the accuracy of PET/CT scans for identifying cancer in the mediastinal lymph nodes of patients with lung tumors smaller than 2 cm.<br /><br />The study involved recording both clinical and pathological mediastinal staging statuses. PET/CT diagnostic performance was evaluated using metrics like sensitivity, specificity, and accuracy. For patients clinically staged as N0, results showed a PET/CT sensitivity of 91.0%, specificity of 32.3%, and accuracy of 82.1%. Meanwhile, for patients staged as N2, the sensitivity was 98.4%, specificity was 78.5%, and accuracy was 88.8%.<br /><br />The findings indicated that while PET/CT demonstrates high sensitivity, its specificity and overall accuracy are lower, particularly when identifying N0 status. In scenarios where PET/CT indicates N0 nodes, the accuracy dropped, emphasizing the importance of further invasive procedures like videomediastinoscopy or video-assisted mediastinoscopic lymphadenectomy (VAMLA) for comprehensive staging.<br /><br />The study concludes that relying solely on PET/CT for mediastinal staging in T1a-b NSCLC is insufficient due to its low specificity. Hence, comprehensive staging, possibly involving surgical interventions, is recommended to ensure accurate detection of N1-2 lymph node involvement. These findings point to the importance of corroborating PET/CT results with additional invasive staging techniques to ensure accurate tumor staging and management.
