P2.05C.02 Does the Proposed 9th TNM Offer Improved Prognostic Accuracy after Lung Carcinoma Resection? Preliminary Analysis Results

The study aimed to evaluate the prognostic accuracy of the proposed 9th edition of the TNM classification for lung carcinoma compared to the 8th edition. This retrospective study involved analyzing medical data of 200 patients who underwent lung resection for primary lung carcinoma between 2012 and 2013. The study sought to compare overall survival (OS) and disease-free survival (DFS) based on the staging according to the 8th and proposed 9th TNM editions.<br /><br />Key findings included:<br />- Of the patients studied, 66% were men and 34% women, with a median age of 64 years. Adenocarcinoma (47.5%) and squamous cell carcinoma (36%) were the most common cancer types, primarily in stages IB, IIB, and IIIA.<br />- A high percentage (96%) of patients achieved R0 resection, indicating no residual tumor following surgery.<br />- No significant differences were found in OS and DFS between the staging groups when reclassified from the 8th to the 9th TNM (OS P-values: 0.73, 0.93, 0.81, 0.49; DFS P-values: 0.41, 0.59, 0.51, 0.18 for stages IIA, IIB, IIIA, and IIIB, respectively).<br />- Stratification according to the 8th TNM showed a significant difference for OS and DFS (P-values: 0.0066 and 0.0031, respectively), whereas the 9th TNM showed weak or no difference (OS P=0.0391; DFS P=0.09).<br /><br />The analysis concluded that the 8th TNM edition more effectively differentiated prognostic staging groups for OS and DFS compared to the 9th edition among their patient cohort. Limitations of the study were noted, including the small sample size and the need for multivariate analysis, indicating the study would continue for more comprehensive results.
