However, the 5-year cumulative incidence of locoregional recurrence was significantly higher in single segmentectomy (17%) compared to multiple segmentectomies (10%) (p=0.03).<br /><br />The researchers used multivariable Cox regression analysis to adjust for high-risk pathologic features and tumor size, finding that multiple segmentectomy remained associated with a lower risk of locoregional recurrence (CSHR 0.39, 95% CI 0.22-0.70). In subgroup analyses, even after adjusting for margin length and other features, this association persisted, though the effect size was reduced (CSHR 0.55, 95% CI 0.31-0.98).<br /><br />The study suggests that while multiple segmentectomy may provide superior locoregional control by ensuring adequate margins, this advantage does not significantly impact overall survival. Further research is necessary to refine the selection criteria for single and multiple segmentectomies to optimize surgical decision-making for patients with early-stage NSCLC.
