P2.11A.14 Research of the Predictive Role of Immune Cells and Intestinal Flora on Immune-related Adverse Event in Lung Cancer

This study investigates the predictive role of immune cells and intestinal flora in the development of immune-related adverse events (irAEs) in lung cancer patients treated with immune checkpoint inhibitors (ICIs). These inhibitors, while effective against advanced lung cancer, can disrupt immune tolerance, leading to irAEs that may necessitate discontinuing or switching medications and can worsen patient conditions.<br /><br />The research involved 88 lung cancer patients receiving ICIs at the Second Affiliated Hospital of Dalian Medical University between May 2021 and September 2023. Peripheral blood and stool samples were collected before treatment and before each treatment cycle to monitor immune cell profiles and intestinal flora through techniques like flow cytometry, metagenomics, and metabolomics.<br /><br />Findings indicated that 41 patients developed irAEs, with a median onset time of 118 days. Those who developed irAEs had unique immune and microbiome profiles compared to those who did not. Specifically, a higher percentage of baseline PD-1 expression in CD8 T cells and distinct variations in B-cell counts and activation states of T cells were observed in patients with irAEs. Furthermore, the intestinal flora differed between groups; Micrococcus was more prevalent in patients who developed irAEs, while Phocaeicola coprocola and Oscillibacter were prevalent in those who did not. Significant metabolic differences were found, with calcitriol and L-isoleucine emerging as potential biomarkers for irAE prediction.<br /><br />In conclusion, monitoring baseline and dynamic changes in immune cells and gut microbiota can help predict and manage irAEs in lung cancer patients, potentially guiding more effective treatment strategies.
