Pdf Summary
The DREAM (Double Lung Transplant Registry Aimed for Lung-Limited Malignancies) trial is a prospective study focusing on bilateral lung transplantation for patients with medically refractory cancers confined to the lungs. Approved by Northwestern University's IRB, the trial commenced enrollment on November 16, 2022, and continues as of September 2024. It aims to explore lung transplantation's applicability for patients with lung-limited metastases and those with respiratory failure and a recent history of cancer.

The study involves three patient cohorts:
- Cohort A includes patients with primary lung cancers, such as multifocal carcinomas and certain non-small cell lung cancers.
- Cohort B includes patients with metastatic cancers that are confined to the lungs, including germ cell tumors and colorectal tumors.
- Cohort C includes patients who have respiratory failure with a history of cancer within the past five years, such as those with interstitial lung disease or chronic obstructive pulmonary disease.

Eligible participants must have no extrapulmonary metastasis, have exhausted standard or experimental oncological treatments, and meet lung transplantation evaluation criteria. The study's goals include evaluating overall survival, disease-free survival, allograft rejection, and survival while monitoring molecular and genetic biomarkers.

The trial, with a 10-year duration and a 5-year recruitment period, aims to enroll 125 participants through Northwestern Medicine Clinical Programs. It involves non-sequential double lung transplantation using a full cardiopulmonary bypass technique to reduce intraoperative cancer cell seeding. The study could provide insights into the feasibility and benefits of lung transplantation for selected cancer patients, similar to liver transplantation for patients with liver-confined metastases.
