P3.13E.01 Is Systematic Lymphadenectomy Pivotal for Bronchial Carcinoid Surgical Treatment? A TNM 9th Classification Validation Pilot Study

The study aimed to evaluate systematic nodal dissection, with or without bench dissection of lymph nodes along segmental bronchial branches, versus nodal sampling according to the 9th pTNM lung cancer staging proposal. This retrospective research utilized data from two institutional databases and employed statistical tests such as the χ2 test, Fisher’s test, and Mann-Whitney test for analysis, with survival analyses conducted using the Kaplan-Meier method and the Log-rank test.<br /><br />The study included 367 patients from February 14, 1983, to November 17, 2023, comprising 297 with typical carcinoid tumors (TC) and 70 with atypical carcinoid tumors (AC). It was found that 9.6% of TC patients and 17.14% of AC patients had metastatic nodal disease according to pathology reports. The Kaplan-Meier analysis indicated a better five-year progression-free survival rate for TC compared to AC, with a statistically significant p-value of 0.0001.<br /><br />The conclusions emphasized the importance of performing systematic lymphadenectomy during surgery for NET (neuroendocrine tumor) lung tumors, regardless of whether the TC or AC histotype is known before surgery (cTNM classification). This approach is crucial to prevent recurrence and prevent disease understaging, which could delay necessary adjuvant therapy. The study suggests that systematic lymphadenectomy plays a vital role in bronchial carcinoid surgical treatment within the context of the 9th TNM classification guidelines.<br /><br />The study was conducted by L. Valentini and colleagues from institutions in Bologna, Perugia, and Forlì, Italy, and supports the pivotal role of systematic lymphadenectomy in treating bronchial carcinoids, validating the TNM 9th classification.
