P4.04C.04 Epidemiological Characteristics of Incidental Pulmonary Nodules and in China: A Prospective Multicenter Trial of 10,560 Cases

The study, conducted in China, examined 10,560 participants with non-calcified pulmonary nodules ranging from 5-30 mm identified via low-dose computed tomography (LDCT) or CT scans. Conducted between October 2018 and May 2021 across 23 clinical centers, the study focused on the prevalence, demographic, and epidemiological characteristics of pulmonary nodules and associated lung cancer risk.<br /><br />Key findings include a higher proportion of female participants (56.6%), with females having a younger median age than males. The study also noted a higher prevalence of smoking and occupational exposure among males. Pulmonary ground-glass nodules (GGNs) made up a large proportion of the nodules, with persistent GGNs (pGGNs) and mixed GGNs (mGGNs) found more often in smaller nodules, females, and non-smokers, while solid nodules appeared more in larger nodules, males, and smokers.<br /><br />Among the participants, 16.0% of high-risk individuals were pathologically diagnosed with 90.35% of malignant nodules detected in very early cancer stages (AJCC Stages 0-I). There was a noted 20% misdiagnosis rate for small and solid nodules. The study further identified that current screening guidelines may miss 13-14% of lung cancer cases.<br /><br />Independent risk factors identified included younger age, female gender, higher education level, multiple target nodules, specific nodule sizes and locations, as well as certain imaging characteristics. The study suggests the potential need to refine high-risk population criteria for more effective lung cancer screening by considering additional epidemiological and clinical risk factors specific to the Chinese population.
