Pdf Summary
The Lung Cancer in those who Never Smoked Study (LuCNeSS) aims to advance lung cancer screening by focusing on individuals who have never smoked. While traditional lung cancer screening using low-dose chest CT (LDCT) in smokers has proven effective, screening for never-smokers lacks established protocols in the U.S., despite promising results for those with a family history of lung cancer in other countries, such as Taiwan.

LuCNeSS is designed to refine and validate lung cancer risk prediction models, known as PLCOall2014 and PLCOall2014bio. These models aim to identify never-smoking individuals at high risk for lung cancer who could benefit from LDCT screening. Key aspects of the study involve examining DNA methylation patterns and epigenomic changes in airway epithelial tissue, which may predict the presence of cancerous lung nodules.

The study plans to enroll 300 participants, including 100 with lung cancer and 200 without. Participants from the Beth Israel Lahey Hospital network will provide detailed medical, environmental, and family history information, as well as biological samples like nasal swabs and tumor tissue, when available. The PLCOall2014 and PLCOall2014bio models will incorporate data on occupational and environmental exposures, with internal validation performed through cross-validation and bootstrap methods.

Preliminary results indicate a diverse range of environmental exposure among participants and potential genetic factors at play in never-smokers with lung cancer. Among diagnosed cases, a mix of cancer types and genetic drivers were identified, with many cases presenting adenocarcinoma.

Continued participant accrual will help refine these models, potentially leading to effective early lung cancer screening strategies for never-smokers, ultimately aiming to reduce lung cancer mortality in this population.
