This study assesses the risk of developing second primary lung cancer (LC) in women who have previously been diagnosed with breast cancer (BC). Conducted at the Icahn School of Medicine at Mount Sinai, the research analyzed data from women aged 54 to 75 who participated in the Prostate, Lung, Colorectal, and Ovarian Cancer Screening Trial (PLCO) between 1993 and 2017. Using a time-updated Cox Proportional Hazards model, the study evaluated the relationship between a history of breast cancer and LC incidence, adjusting for factors like age, race, education, and smoking patterns.<br /><br />The results indicated that women with a history of breast cancer had a 22% increased risk of developing LC compared to those without BC (AHR: 1.22, 95% CI: 1.01, 1.47). Current smokers had an even higher risk of LC, nearly double that of never-smokers (AHR: 1.96; 95% CI: 1.25, 3.08). The findings suggest that BC survivors might benefit from tailored interventions for LC prevention, such as smoking cessation and low-dose CT screening.<br /><br />Despite common risk factors shared by both cancers, such as smoking and age, a history of BC itself emerged as an independent risk factor for LC. Furthermore, higher education levels and BMI were inversely related to the risk of LC, indicating potential protective factors. The study emphasizes the need for individualized screening and prevention strategies for BC survivors and calls for further research to optimize LC screening protocols to ensure benefits outweigh harms. Overall, the study diagnosed 218 LC cases among BC survivors, yielding an unadjusted incidence rate of 242 per 100,000 person-years. This research highlights important considerations for the medical management and follow-up of breast cancer survivors.
