P4.07E.10 The Impact of PD-1/PD-L1 Inhibitor on GGN Lesion: An Exploratory and Retrospective Study

This study investigates the effects of PD-1/PD-L1 inhibitors on ground-glass nodules (GGNs), early indicators of lung cancer detectable via CT imaging. While immunotherapy has notably improved outcomes for advanced lung cancer patients, its impact on GGNs, particularly when these are present as multiple primary lung cancers, is not well-documented.<br /><br />The research focuses on patients with primary malignant tumors coexisting with GGNs who received at least two cycles of PD-1/PD-L1 inhibitors. Study criteria included patients who had a baseline thin-slice chest CT before treatment, GGNs measuring between 4 to 30 mm, and follow-up CT imaging conducted within a month after completing immunotherapy. <br /><br />Involving 76 GGNs from 39 patients, the study found that 74.6% of GGNs (classified as Lung-RADS 2 or 3) remained unchanged post-treatment. Additionally, 80% of GGNs classified as Lung-RADS 4 were downgraded. The objective response rate of GGNs was 10.5%, with partial response observed in 8 GGNs, stable disease in 61, and progressive disease in 7. Overall, the response rate for primary malignant tumors was 36.8%, and 44.7% of GGNs displayed a response consistent with the primary tumors' response.<br /><br />Aspects such as GGN maximum diameter and volume correlated positively with the immunotherapy response. GGNs exceeding 8 mm and those with a volume greater than 70 mm showed potential benefit from PD-1/PD-L1 inhibitors. GGN density and volume emerged as independent predictors of treatment efficacy, indicating that those with larger volume achieved more significant results.<br /><br />Thus, PD-1/PD-L1 inhibitors exhibit limited activity on GGNs in malignant tumor cases, with larger GGNs demonstrating some potential benefit from the treatment.
