This study investigates the impact of different morphological types of ground glass opacity (GGO) on patient outcomes in solid-dominant peripheral lung adenocarcinoma. Conducted at Zhongshan Hospital, Fudan University, Shanghai, it evaluates the prognosis associated with central, scattered, and eccentric island-shaped consolidations in lung adenocarcinoma. The study retrospectively analyzed 200 patients who underwent radical surgery from 2013 to 2015. The preoperative computed tomography (CT) scans categorized the patients into 170 with central island-shaped consolidations, 18 with scattered shapes, and 12 with eccentric shapes.<br /><br />Outcomes were measured in terms of recurrence-free survival (RFS) and overall survival (OS). The results showed significant differences in survival between patients with central island-shaped consolidations and those with scattered or eccentric shapes. The Kaplan–Meier survival analysis indicated that patients with central island-shaped configurations had a 5-year OS of 100% compared to 92.1% for scattered or eccentric shapes. Similarly, the RFS was substantially higher for the central type (100% vs. 59.7% for c-stage IA, and 100% vs. 64.5% for p-stage IA).<br /><br />Univariable logistic regression analysis revealed that patients with scattered and eccentric GGO shapes had a poorer prognosis, with an odds ratio of 18.8 (p = 0.012). Eleven patients in the study experienced recurrence or metastases, further underscoring the potential risk associated with non-central morphological patterns.<br /><br />The research concludes that the spatial relationship between GGO and solid components significantly affects patient outcomes. Specifically, patients with scattered or eccentric shaped GGO are at higher risk for recurrence and have a worse prognosis compared to those with more central configurations.
